The quick intervention of a medical case manager is a proven and effective service that reduces employer lost-time costs due to workplace injuries.

After injury onset, the coordination and follow-up of medical treatments are key components to a cost-effective return to work process. Medical case management assists with the coordinating, scheduling and monitoring of the injured worker's appointments with medical, rehabilitative, and other services. This helps to ensure the injured worker has timely and coordinated access to medically appropriate levels of health and support services, and continuity of care through our ongoing assessment of the client's needs, while controlling the claim costs and ensuring treatment goals are met.

VocMed's medical case management services are client-centered services which assist to ensure the timely implementation of a treatment plan tailored to the injured worker's needs, with the goal of returning to the workforce in as expeditious, yet safe, a manner as possible. Injured workers see medical case managers as caring and compassionate and, as such, our nurse case managers can build relationships with the client, employer, and physician to develop a care plan suited to the patient which will balance the clinical, functional, vocational, and often emotional aspects as part of a holistic approach to the Workers' Compensation driven medical care.

VocMed employs only the most experienced nurses, with decades of experience in providing services to the disabled populace through the workers' compensation system. All of our medical case management team is at minimum a Registered Nurse, with many having Bachelor's degrees and above. We strive to achieve only the most positive clinical outcomes, and our nurses handle all claims in the most professional, ethical, and expeditious manner possible.

Initial Intervention

Telephonic Medical Case Management

When an on the job injury occurs, telephonic case management is the initial step in ensuring the injured worker receives the proper care, while also decreasing long-term costs to the insured.

Telephonic case management is not a part-time job. The average VocMed telephonic nurse case manager is a Registered Nurse with decades of experience in handling medical claims for the disability industry. Upon referral, our medical case manager will make immediate three-point contact with the client, physician(s) and the employer, to obtain an up to the minute status of the case. Our case manager will then assist in the development and implementation of an individualized treatment plan, aimed at returning the injured worker to the workforce as quickly as possible.

VocMed's medical case management team will continue the client's care by coordinating and monitoring appointments to, and working in conjunction with, all appropriate medical care providers to assist in the success of the treatment plan. As part of the treatment plan, we will follow up with the employer and physician and develop a return to work program, tailored to the injured worker's needs, and designed to return the injured worker to his or her pre-injury job as safely and promptly as possible.

On-Site Support

Field Medical Case Management

Depending on a variety of factors, some medical case management cases may require direct nurse case management intervention to assist in safely returning an injured worker to the workforce.

VocMed's nurse case managers will ensure that the provided medical care is both appropriate and cost-effective. Upon referral, our medical case manager will make immediate three-point contact with the client, physician(s) and the employer, to obtain an up-to-the minute status of the case. Our nurse case manager will then meet with the client and treating physician in person and assist in the development and implementation of an individualized treatment plan, aimed at returning the injured worker to the workforce as quickly as possible.

VocMed's medical case management team will continue the client's care by coordinating and attending appointments to, and working in conjunction with, all appropriate medical care providers to assist in the success of the treatment plan. As part of the treatment plan, we will meet with the employer and physician and develop a return to work program, tailored to the injured worker's needs, designed to return the injured worker to his or her pre-injury job as safely and promptly as possible.

Getting Back to Work

RTW Coordination and Modification

An appropriate return to work program involves careful coordination and close communication between the injured worker, employer, healthcare provider(s), and other stakeholders in the disability claim. Effective return to work programs have been proven to not only reduce claim costs, but also assist in keeping the injured worker engaged in the rehabilitation process and remain motivated to return to the workforce.

When an injured worker has reached a stage of recovery where it is appropriate to safely return to work, VocMed's medical and vocational case management teams will coordinate with all stakeholders to arrange the return to work. When possible, we return injured workers to their pre-injury jobs safely and promptly. Should the injured worker be unable to return to their pre-injury job, our nurse case managers will work with the employee, healthcare provider(s), and employer to identify additional suitable positions, or identify appropriate modifications to the original position, on the injured worker's behalf.

Complex Care

Catastrophic Case Management

Catastrophic injuries present both immediate and long-term cost risks for an employer, as well as continued physical and emotional risks for the injured worker. Immediate dispatch of our nurse case managers to the trauma center or facility ensures a swift on-site assessment and report to the account, appropriate and timely care for the injured worker, as well as expressing care and concern for the patient and family. Upon involvement, our catastrophic case managers will conduct an immediate assessment of the injured worker's condition and care, as well as begin coordinating both the immediate and long-term care required.

As most catastrophic claims are complex and long-term in nature, without proper management, recovery can be prolonged and more costly than necessary. Thus, it is imperative to have an experienced catastrophic nurse case manager to coordinate the multiple providers and specialists, often who aren't in contact with each other, and thus able to formulate a long-term plan tailored to the injured worker's medical needs. Not only will this reduce claims costs, but will also best help the injured worker achieve independence and a quality of life after their debilitating injury.

Clinical Clarity

Independent Medical Exam | Panel Coordination

On difficult cases there are times when it is important to clarify a diagnosis and treatment plan. An IME can provide information regarding the cause of an injury, medical condition, medical relatedness, treatment alternatives, or the disability duration of a claim.

Our nurse case managers will assist in identifying the highest quality board certified specialists who can assess the claim from their area of expertise, and render an opinion as to return to work options, length of disability, extent of disability, and anticipated maximum medical improvement.

When requested, we will work with a referring agency to establish a panel of physicians that will then be provided to the injured worker. Once a physician is selected, we will coordinate scheduling of the medical appointment.
